Nathan Brimm has been our father’s care manager for close to three years. Our father suffers from vascular dementia, a difficult and unpredictable disease. As we have come to know Nathan, he has shown great empathy and compassion for our father balanced with an ability to meet the inherent changes in his disease head on. His clinical experience as an RN, BSN as well as his expertise in dealing with people who can be difficult has proven to be a great source of support for our family. His kind and gentle yet firm guidance for our father has been critical as our dad moves through this disease. His number one goal is our father’s quality of life and safety. We are grateful for his guidance and suggestions. His recommendations have helped immensely to ease the burden of watching how dad’s disease has and will progress. I was POA/guardian for a friend who was genuinely alone, living with progressive dementia and multiple other chronic conditions. Unfortunately, I lived 700 miles away. I was so grateful when I was referred to Nathan for ongoing care coordination and advocacy across disconnected care providers and facilities. Nathan is an excellent problem solver, initiates needed actions to solve problems, and follows up to ensure things are done well and with compassion. While the notion of 'patient/client-centered care 'is voiced by many organizations, it is pragmatically bounded by the overarching structure of the organizations' business model. In contrast, Nathan's business model is person- and family-centered care managed across time, providers, and facilities. For over two years, as my friend’s condition declined, Nathan accompanied my friend to appointments, skillfully advocated for approaches that maintained the quality of life as expressed by my friend, and then coordinated care delivery and communication whether my friend was in independent living, assisted living, skilled nursing, or hospice. He was very knowledgeable about other hi-quality services in the area, e.g. bill-paying services, legal services, etc. As noted in other reviews, Nathan provided "kind and gentle yet firm guidance" to me and others. He maintained close communication even though we were remote; I was always confident that he had a set of eyes on the details and the big picture. Dignified Aging Strategies filled a tremendous service gap for my friend and me.
